	Every eligible applicant MUST complete the form separately. Each eligible member will receive their own individual Loyalty Member number.
	All applicants must be at least 21 years of age to qualify.
	All information entered MUST pertain ONLY to sailings that were booked and COMPLETED through CRUCON.
	Please provide ALL of the sailings that you have COMPLETED through CRUCON. (including the CRUISE LINE, SHIP and DATE)

Please fill out the form in its entirety. Please fill in as many cruises as you can remember, that you have COMPLETED through CruCon within the last five (5) years. This completed form will ensure the accuracy of your information within our database.

Note: Shortly after you are a validated Loyalty Member, you will receive a BRAND NEW Loyalty Club “Welcome” package in the mail (USPS).
